Hi. Just wondering if you could show an example of a worksheet with incompetent veins and incompetent perforators, to see how you document everything you discussed in this talk?

Here are the worksheets we use,

They are nothing special,

I think it is important to be descriptive and also draw the venous patterns so there is a graphical representation that is easy for the referrer to follow and understand.

I will often add a blank worksheet with an additional hand drawn diagram,
