      What is this hypoechoic area?
      Is this tonsil?
      Is this normal or abnormal?

      SMG: submandibular gland

      This is a bit tricky!

On your long axis images I think it looks like tonsils,

However on the axial image I am not certain.

The tongue should be echogenic on ultrasound as the muscle of the tongue is very fibrous in nature. On your axial image the hypo echoic area spans from right to left across the midline.
This is not what I expect from tonsils.
      Also it is quite vascular.
What was the patients clinical presentation?

I have diagnosed several tongue cancers on ultrasound over the years and I think a CT scan or MRI may be worthwhile to make sure we are not seeing something sinister.

      Safety first!

      Steve.
