I would like to say yes, as the process of gynaecomastia is exactly the same as a female breast at puberty. The only problem I have is your patients age as by 17 – 20 years of age the breast should be well developed and not just starting the process. I have certainly seen this “gynaecomastia” appearance in a female breast at the age of puberty and indeed it can feel firm and be tender (like a male gynaecomastia case), however I have not seen this appearance at 17 – 20 years of age.
